Welcome to the website of Healique Health, LLC (“Healique Health,” “we,” “us,” or “our”). These Terms of Use govern your access to and use of the Healique Health website, online scheduling features, patient-facing resources, and other website-based services.

By accessing or using this website, you acknowledge that you have read, understood, and agree to these Terms of Use. If you do not agree with these Terms, please do not use this website.

About Healique Health
Healique Health is a healthcare practice offering select virtual and mobile healthcare services.
Services may include:

  • Telehealth urgent care

  • Medical weight management

  • Medical cannabis evaluations

  • Emotional Support Animal (ESA) evaluations

  • School, sports, employment, and other eligible physical examinations

  • Mobile and on-site physical events

Availability of services may vary based on location, patient eligibility, provider availability, applicable laws and regulations, and clinical appropriateness.

Healique Health does not currently provide comprehensive primary care services. Patients are encouraged to maintain an established primary care provider for routine preventive care, chronic disease management, and other ongoing healthcare needs.

Website Information Is Not Medical Advice

Information provided on this website is intended for general informational and educational purposes only. Website content should not be considered a substitute for individualized medical advice, diagnosis, treatment, or evaluation by a qualified healthcare professional.

Do not disregard professional medical advice or delay seeking medical care because of information you have read on this website.

Use of this website alone does not establish a provider-patient relationship between you and Healique Health or any Healique Health provider.

A provider-patient relationship is established only when applicable requirements have been satisfied and a Healique Health provider has agreed to provide clinical services to you.

Medical Emergencies

Healique Health is not an emergency medical service. Do not use the website, patient portal, email, voicemail, online scheduling system, or telehealth services for a medical emergency.

If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency department immediately. Examples may include severe chest pain, significant difficulty breathing, signs or symptoms of stroke, loss of consciousness, severe bleeding, serious injury, or other potentially life-threatening symptoms.

Telehealth Services

Healique Health may provide certain healthcare services using telehealth technology. Telehealth may involve electronic communications between you and your provider, which may include video, audio, secure messaging, electronic health information, photographs, questionnaires, or other technology when clinically and legally appropriate. Telehealth is not appropriate for every medical condition.

During or following an evaluation, your provider may determine that an in-person examination, laboratory testing, imaging, specialist evaluation, urgent care, emergency care, or another service is necessary. You agree to provide accurate information regarding your identity, physical location at the time of the visit, medical history, medications, symptoms, and other information reasonably necessary for your care. Clinical services are available only when Healique Health and its providers are legally authorized to provide the requested care in the jurisdiction where the patient is physically located.

Telehealth Urgent Care

Telehealth urgent care is intended for appropriate non-emergency medical concerns that can reasonably and safely be evaluated remotely. Not every condition can be diagnosed or treated through telehealth. A Healique Health provider may decline to treat a condition virtually or recommend additional evaluation when clinically indicated. A telehealth urgent care visit does not guarantee a diagnosis, prescription, antibiotic, work or school note, laboratory order, referral, or any particular treatment. Healique Health's urgent care services are not intended to replace comprehensive primary care, ongoing management of complex chronic conditions, or emergency medical care.

Prescriptions and Medications

A consultation with Healique Health does not guarantee that medication will be prescribed. All prescribing decisions are made by the treating provider based on clinical judgment, medical necessity, applicable professional standards, and federal and state law. Your provider may determine that a medication is inappropriate, that additional information or testing is necessary, or that an in-person evaluation is required before prescribing. Healique Health does not guarantee that a particular medication will be available, covered by insurance, approved through prior authorization, or dispensed by a pharmacy.

Medication and pharmacy costs are separate from Healique Health's professional fees unless expressly stated otherwise.

Medical Weight Management

Medical weight management services are individualized based on factors including medical history, current health status, medications, weight history, treatment goals, and clinical findings.

Participation in a weight-management evaluation or program does not guarantee:

  • Eligibility for a particular medication;

  • Prescription of a GLP-1 or other weight-management medication;

  • Insurance coverage or prior authorization;

  • A particular amount or rate of weight loss; or

  • Any specific treatment outcome.

Laboratory testing or additional medical evaluation may be required before or during treatment.

Medication, laboratory, pharmacy, imaging, and other third-party costs are separate unless specifically identified as included in the applicable service price.

Medical Cannabis Evaluations

Healique Health may provide medical cannabis evaluations for patients seeking certification under the Maryland Medical Cannabis Program, subject to applicable Maryland requirements. Payment for an evaluation is payment for the provider's professional time, clinical assessment, and related services. Payment does not guarantee medical cannabis certification. Certification will be provided only when the evaluating provider determines that the patient satisfies applicable eligibility requirements and that certification is clinically appropriate.

Healique Health does not sell, dispense, distribute, or guarantee access to cannabis products. Patients are responsible for complying with applicable Maryland laws, regulations, registration requirements, and program rules. Medical cannabis may have risks, side effects, medication interactions, and implications for activities such as driving, employment, professional licensing, firearm possession, and compliance with federal law. Patients are responsible for understanding requirements that may apply to their individual circumstances.

Emotional Support Animal Evaluations

Healique Health may provide clinical evaluations for individuals requesting documentation concerning an Emotional Support Animal (ESA).

Payment is for the professional clinical evaluation and does not guarantee that an ESA letter, form, certification, or other documentation will be issued.

Documentation will be provided only when the evaluating provider determines that it is clinically supported and professionally and legally appropriate.

Healique Health does not guarantee that a landlord, housing provider, employer, airline, educational institution, government agency, or other third party will accept any documentation provided. An Emotional Support Animal is not necessarily considered a service animal under laws applicable to service animals, and different legal protections and requirements may apply depending on the setting.

Physical Examinations and Mobile Services

Healique Health may provide eligible physical examinations, including school, sports, employment, general, and DOT physical examinations, as well as organized mobile or on-site physical events. Completion of an examination does not guarantee medical clearance, participation eligibility, employment clearance, DOT certification, or completion of a requested form. Clinical determinations will be based on examination findings, relevant medical information, and applicable standards or requirements. If additional testing, laboratory services, vaccinations, imaging, specialist evaluation, or other services are needed, patients may be referred to an appropriate outside provider or facility.

Mobile and on-site services are subject to geographic availability, scheduling, facility suitability, and other practice requirements. Additional mobile or convenience fees may apply and will be disclosed when applicable.

Appointments and Scheduling

Appointment availability is not guaranteed. Healique Health may reschedule, cancel, modify, or decline an appointment when reasonably necessary because of provider availability, technical difficulties, patient eligibility, clinical appropriateness, safety concerns, or other circumstances.

Patients are responsible for providing accurate contact information and monitoring communications regarding scheduled appointments.

Patients should review Healique Health's current cancellation and no-show policy before scheduling.

Fees, Payment and Insurance

Current self-pay rates are available on the Healique Health Services & Pricing page or through the applicable scheduling system. Payment will be required before or at the time services are provided.

Unless expressly stated otherwise, Healique Health's fees cover the professional services provided by Healique Health and do not include charges from third parties, such as:

  • Prescription medications

  • Pharmacy services

  • Laboratory testing

  • Imaging

  • Vaccinations

  • Specialist consultations

  • Hospital or emergency services

  • Outside medical facilities

  • Other third-party services

Insurance participation and coverage may vary by service and health plan. Patients are responsible for understanding their individual insurance benefits, coverage limitations, deductibles, copayments, coinsurance, and other financial obligations. Healique Health does not guarantee payment or reimbursement by any insurer or health plan.

Refunds and Clinical Outcomes

Payment for a professional evaluation compensates Healique Health for the clinical services performed and does not purchase or guarantee a specific outcome. Unless otherwise required by law or stated in an applicable refund policy, fees for completed professional services are not refundable merely because a patient does not receive a requested prescription, certification, ESA document, medical clearance, referral, or other desired outcome.

Any cancellation, refund, or rescheduling rules applicable to a particular service will be communicated through Healique Health's applicable policies or scheduling process.

Patient Responsibilities

By using Healique Health's clinical services, you agree to provide complete and accurate information to the best of your knowledge.

This includes information concerning your:

  • Medical and surgical history;

  • Pregnancy status when relevant;

  • Previous diagnoses and treatments; and

  • Other information reasonably requested for your evaluation.

Withholding or providing inaccurate medical information may affect the safety and quality of your care.

Electronic Communications

Healique Health may communicate with patients electronically regarding scheduling, administrative matters, reminders, billing, and healthcare services as permitted by law and applicable patient preferences. Ordinary email and text messaging may not always be appropriate for communicating sensitive medical information. Patients should use Healique Health's designated secure patient portal or other approved communication method when instructed to do so.

Electronic messages should not be used for emergencies.

No Guarantee of Results

Healthcare outcomes vary among individuals.
